Build the form once, then reuse it across as many activities as you like.
Go to Settings → Waivers.
Click New waiver.
Give it a name and, optionally, a description — participants will see both when they sign.
Click + Add New Field and choose a field type from the picker (see the full list below). Repeat for every field you need.
Reorder, duplicate, or remove fields at any time — the form updates live as you build it.
Use Preview to see exactly what your customer will see before you publish.
Save. New waivers are active by default — switch a waiver to inactive if you're not ready to use it yet.
Available field types

Short Text | A single-line answer, e.g. a name. |
Paragraph | A longer free-text answer. |
A validated email address. | |

Date | A date picker, e.g. date of birth. |
Dropdown | Pick one option from a list. |
Single Choice | Radio-button options. |
Checkboxes | Pick any number of options. |
Address | A structured mailing address. |
Emergency Contact | Name, phone, and relationship. |
Legal Acknowledgment | A clause the participant must check off, e.g. a release of liability. |
Signature | Draw-to-sign, captured as an image. |
Info Notice | Read-only note — no input required. |
Warning | Read-only, styled to stand out — for anything risk-related. |
Photo Upload | Participant attaches a photo, e.g. ID or medical card. |
